That thing happens every day, every time, just a different map, so today it striked this map, not a big deal, I don't think the admins can do anything about it

Yea, unfortunately map entities just go crazy sometimes, either not spawning or not respawning. It'd be something Valve would have to fix, but their priorities aren't in hunting down very obscure and rare bugs.
